New Patient Packet
If you and your child(ren) are new to our practice and have an upcoming appointment, please fill out our New Patient Packet. This packet includes: the Patient Information form, Medical/Dental History form, HIPAA Notice of Privacy Practices, our Financial & Appointment Policies and a Covid-19 Pre-screening Questionnaire.
